3-PAR CSV disk goes offline in Hyper-V windows 2012 R2 setup

Team,
We have 3 -Node Hyper-V Cluster Windows 2012 R2.
Problem is CSV disks , which goes offline for any small IO carried on it. Copy etc.
This is DR site for Data replication from DC. At present no replication is carried out. Everything has come to a stand still.

Had a query , i have already disabled ODX. OS is 3.1.2 . Have asked HP to upgrade to latest one.

Note :At DC we have XtremIO & EMC VNX5500 and have no issues with CSV disks. But 3PAR is giving restless nights at DR.

I would start with the OS upgrade. 3.1.2 is old (released in 2012), the first release to support ODX and end of life.

Something in the back of my head tells me I've read something about this before, so you could be hitting an old issue which has been covered in some form of advisory.
